Community Impact Grant Agency Application

Complete applicaitons must be recieved by United Way Montcalm-Ionia Counties, 10260 S. Sheridan Rd, Fenwick, MI 48834, no later than 12:00 PM on December 20, 2018.  No late submissions will be accepted. 

All documents can be obtained by clicking on the titles below.  There are no downloadable documents for Attachment D or Attachment E. These are documents that you will provide with your completed application for funding.

RFP Instructions

The following must be completed and returned with application for funding:

Check List

Provide one (1) copy of Check List with completed application for funding.

Cover Page - Attachment A

Provide eleven (11) copies of Attachment A with your completed application for funding.

Capacity To Deliver - Attachment B

Provide eleven (11) copies of Attachment B  with your completed application for funding.

Nondiscrimination and Anti-Terrorism Compliance - Attachment C

Provide two (2) copy of Attachment C with your completed application for funding.

Attachment D

Financials  (Audit or 990) and Accountability documents. Provide two (2) copies Attachment D at time of submission.

Attachment E

Organization or program logo.  Agency provides Attachment E at time of submission.  Please email your logo to grants@liveunitedm-i.org.

Attachment F
